You may request a Benilde Discipline Certificate, which is our equivalent of a Certificate of Good Moral Character, by answering the Benilde Discipline Certificate Request Form.
You can claim your certificate in the Center for Restorative Discipline, Taft Campus Office, in Room D305, within 5 working days. The certificate is Php 100 per copy.
CRD will email you once your certificate is ready for pick-up. You need to pick up the certificate in person. If someone else will claim your certificate, they need to present an Authorization Letter.
