Medical Guidelines for Incoming High School Students

For incoming students for AY 2026-2027, you can start submitting your medical clearance and test results by May 15, 2026. The AY 2026-2027 BSHS & BDS Student Medical Record form will be open and accept submissions on this date.

All incoming Benilde High School students must undergo a medical examination at any DOH-accredited clinic. Follow these steps:

  1. Download and print the DLS-CSB Medical Clearance Form. Your parent or guardian should only fill out the Name of the student, the form will be answered by your attending physician.
  2. Proceed to any DOH-accredited clinic or your attending physician for a physical examination and laboratory test. Bring your corrective lenses or wear your soft lenses on the day of your medical examination.
  3. Ask your attending physician to complete the DLS-CSB Medical Clearance Form. 
  4. If you were referred to a specialist after receiving the initial medical evaluation, secure a medical clearance from the specialist that indicates you’re fit to participate in school activities and note any limitations you may have.
  5. Once completed, your parent or guardian should upload the requirements to:
Complete and use the official DLS-CSB Medical Clearance Form when obtaining clearance. Ensure the attending physician has ticked the appropriate recommendations indicating you’re fit to enroll
