Medical Guidelines for Incoming Undergraduate, Graduate, and ETEEAP Students

All incoming Benilde undergraduate and graduate students must undergo a medical examination in any DOH-accredited clinic. Follow these steps:

1. Download and print the DLS-CSB Medical Clearance Form.
2. Proceed to any DOH-accredited clinic or your attending physician for a physical examination and laboratory test. Bring your corrective lenses or wear your soft lenses on the day of your medical examination.
3. Complete the following laboratory tests before your medical exam:
   • Chest X-ray
   • CBC
   • Urinalysis
   • Fecalysis
   • 2-panel drug test 
   • Quantitative Hepatitis Profile/Results (HBsAg, anti-HBs, anti-HAV IgM, anti-HAV IgG) and Hepatitis A and B vaccines as recommended by our School Physician, based on the submitted Hepatitis quantitative profile test results (for School of Hotel Restaurant, and Institution Management students)
   • ECG result (for varsity students)

You can submit laboratory tests taken within the last 3 months.

4. Ask your attending physician to complete the DLS-CSB Medical Clearance Form. 
5. If you were referred to a specialist after receiving the initial medical evaluation, secure a medical clearance from the specialist that indicates you’re fit to participate in school activities and note any limitations you may have.
6. Once completed, upload the requirements to:
   • AY 2026 - 2027 Student Medical Record (ID 126)

• DLS-CSB Medical Clearance Form
• Medical clearance for students recommended for further medical evaluation
• Chest X-ray result
• Laboratory Results (CBC, Urinalysis, Fecalysis)
• Drug Test Result
• Quantitative Hepatitis Profile/Results (HBsAg, anti-HBs, anti-HAV IgM, anti-HAV IgG) (for School of Hotel, Restaurant, and Institution Management students)
• ECG result (for varsity students) 

Complete and use the official DLS-CSB Medical Clearance Form when obtaining clearance. Ensure the attending physician has ticked the appropriate recommendations indicating you’re fit to enroll
