Consult with School Physician/Dentist

For consultation with the School Physician and Dentist, you may either visit the clinic if you are on campus or fill out the CHMS Consultation Form for online consultation. School Clinics are located at:

  • Taft Campus: Greenway Square
  • AKIC Campus: 7th Floor Room 714
  • D+A Campus: 3rd Floor Room A303
  • The Atrium@Benilde: 3rd Floor

Dental Consultation and Schedule:

  • Taft Clinic: Monday to Friday, 9:00 AM - 1:00 PM
