If your class, student organization, school, or office plans to conduct an outreach activity, submit your proposed outreach activity through the Community Outreach Proposal Form:

- School Community Outreach Proposal Form
- Office Community Outreach Proposal Form
You must submit the proposal at least 1 to 2 months before your planned outreach activity.
The Center for Social Action will contact you regarding the details of your proposal. CSA is the main office in charge of building and maintaining partnerships with communities and different organizations for community engagement. Outreach programs should be organized with CSA and our community partners and organizations.
After conducting the outreach activity, fill out and submit the Community Outreach Report Form:
- Class Community Outreach Report Form
- Student Organization Community Outreach Report Form
- School Community Outreach Report Form
- Office Community Outreach Report Form
