These confirmation guidelines are for incoming First-year College Students for First Term AY 2024 -2025.
Once you’re Accepted or Conditionally Accepted, you can secure your slot in the program you’ve applied for by following the confirmation process. The reservation for the degree program slot is on a first-come, first-served basis. Follow these steps to do this:
- Pay the Php 5,000.00 non-refundable reservation fee to your preferred Admission Payment Channel.
- Send your proof of payment in the Benilde Undergraduate Confirmation Form. Your slot won’t be reserved if you aren’t able to answer this form.
- Submit a hard copy of the Enrollment Requirements to the Center for Admissions Office. You can find them in:

Make sure to pay the reservation fee on or before the confirmation deadline. Not being able to do this may mean losing your slot in the program you got accepted into. The confirmation fee is not refundable and will be deducted from your total enrollment fee.
If you plan to request a Change of Program, do this before paying the Confirmation Fee. Once you have paid this, you can no longer change your program.
Enrollment Requirements
Here is the list of the requirements you need to submit to the Center for Admissions after paying the reservation fee. Check if you have your complete requirements with you before going to the Admissions Office.
Filipino First-Year College Applicant
- Original Philippine Statistics Authority (PSA) Birth Certificate
- Original Grade 12 Senior High School Report Cards (F-138) (1st Semester/1st Trimester) duly signed by the principal or school head
- Original Certificate of Good Moral Character
- Download and fill out the following Benilde Enrollment Forms:
- Statement of Responsibilities
- Non-Fraternity Contract (Notarized in any Notary Public)
- Data Privacy Consent Form
- Consent to Release Student Information
- Foreign Language (applicable only to AB-DIA accepted applicants)
- For BS-CAM, BS-HRIM, BS-IHM, and BSTM student applicants, Photocopy of Quantitative Hepatitis Titer Results (HBsAg, anti-HBs, anti-HAV IgG, and anti-HAV IgM). This can be done in any reputable clinic or hospital of your choice. A student may initially be accepted into the program but may later be found ineligible based on the medical diagnostic results and will be asked to change to a different program in DLS-CSB. Print and give the Quantitative Hepatitis Profile Request Form to your chosen service provider.
International First-Year College Applicant
Requirements for tourists and other types of visa holders including permanent residents, dual citizens, and non-native-born applicants
- Original Philippine Statistics Authority (PSA) Birth Certificate (For those born in the Philippines)
- Original Birth Certificate or Family Register (For those not born in the Philippines)
- Original Grade 12 Senior High School Report Cards (F-138) (1st Semester/1st Trimester) duly signed by the principal or school head
- Original High School Transcript of Records (TOR) officially translated in English and duly authenticated or apostille stamp by the Philippine Foreign Service Post (PFSP) of the school’s country of origin (For international applicants and Filipinos who graduated or studied in schools overseas)
- Original Certificate of Good Moral Character
- Download and fill out the following Benilde Enrollment Forms:
- Statement of Responsibilities
- Non-Fraternity Contract (Notarized in any Notary Public)
- Data Privacy Consent Form
- Consent to Release Student Information
- Foreign Language (applicable only to AB-DIA accepted applicants)

- Photocopy of the following documents for verification:
- Valid Passport: Bio page and stamp of latest arrival page
- Valid Alien Certificate of Registration Identification Card (ACR I-Card)
- Valid Identification Card (for SIRV, SRRV, and PRA Holders)
- Identification Certificate or Certificate of Recognition (for Dual Citizens & for those not born in the Philippines)
International student applicants with Tourist Visas, Special Study Permit, and Balikbayan will submit the following documents for student visa processing:
- Checklist of Student Visa Conversion
- Special Study Permit Requirements
- Bureau of Immigration Consolidated General Application Form
- Personal History Student Form
- DLS-CSB Affidavit of Financial Support and Guarantee
The Office of the Registrar will assist confirmed international students in getting their student visas. Only international students with complete visa documents are allowed to enroll. You won’t be allowed to enroll without a student visa.
