A Medical Certificate is a document that indicates a patient’s health condition. School Medical Certificates are issued only to those students and associates who were seen and examined in the DLS-CSB School Clinic.
A Medical Clearance is a document that states fitness to go back to work or class.
Students and associates who were absent due to medical reasons for 3 days or more are required to submit a medical clearance from their attending physician. This has to be presented to the DLS-CSB Clinic for validation by the Physician on Duty. For absences of less than 3 days, submission of a medical clearance will depend on the assessment of the School Physician. This is required to be able to secure a School Medical Clearance before being allowed to go back to work or class.
