If you lost your ID, you will need to submit an Affidavit of Loss to the Center for Restorative Discipline (CRD) before you can get a replacement. You can get this from any Notary Public outside the school. Simply inform them that you need an Affidavit of Loss for your school ID. There is no required format for this. Once you have secured this, go to the Center for Restorative Discipline Office to submit this:
- Taft Campus: D305
- AKIC Campus: I902
- D+A Campus: A918
- The Atrium@Benilde: Mezzanine
CRD is open from Monday to Friday, 8:00 AM - 9:00 PM, and Saturday, 8:00 AM - 5:00 PM. For the Taft Campus, the office hours are Monday to Friday, 8:00 AM - 5:00 PM.
