Students
If you lost your ID, go to the nearest Notary Public and secure an Affidavit of Loss. There is no required format for this. Once you get it, scan and upload it in the Affidavit of Loss form. The Center for Restorative Discipline will email you for the next steps.
If you lost it from theft, robbery, or other similar incidents, submit a Police or Barangay Incident Report along with the Affidavit of Loss.
For damaged or defaced IDs, go to the Office of the Registrar for ID replacement.
If you have your ID but it’s deemed invalid when you enter the school, settle any remaining balances with the Finance Department. You can also check if you have any pending clearances.
Associates
If you lost or damaged, or if your ID is deemed invalid when you enter the school, go to the People and Organization Development Office (PODO). A part-time faculty can enter the school if they have a teaching load for the current term. Consultants can access the campus during the duration of their contracts.

Violations of school ID policies can be reported to Filing of Complaint/Reporting an Incident for students and to PODO at podohelpdesk@benilde.edu.ph for associates.
